ENROLLING YOUR CHILD

Required Documents for Enrollment:

1. Original Birth Certificate for each child you want to enroll (or certified copy with certificate number issued by the State)
2. Immunization (Shot) Records for each child you want to enroll
3. Social Security Card for each child you want to enroll, if available
4. Parent’s Driver’s License
5. Proof of Residency in Graham ISD*

*The following documents are acceptable for Proof of Residency. You only need ONE of these documents for Proof of Residency.*

• A current water bill OR a statement from the City of Graham listing your name and address for water services
• A current natural gas bill with your name and address listed
• A current electricity bill with your name and address listed
• A signed lease agreement with your name and address listed
