ENROLLING YOUR CHILD

You may come by the Crestview Elementary Office for enrollment forms. You may also download and print enrollment forms and return the forms to Crestview, along with the documentation listed below.   

THE FOLLOWING DOCUMENTATION IS NEEDED FOR ENROLLMENT:

• Original Birth Certificate for each child you want to enroll (or certified copy with certificate number issued by the State)
• Immunization (Shot) Records for each child you want to enroll
• Social Security Card for each child you want to enroll, if available
• Parent or Guardian ID (Driver’s License, Passport, or State ID)
• Proof of Residence – one of the following:
  • A current water bill OR a statement from the City of Graham listing your name and address for water services
  • A current natural gas bill with your name and address listed
  • A current electricity bill with your name and address listed
  • A signed lease agreement with your name and address listed
• Withdrawal form from previous school district
• If not custodial or legal guardian, you will need to complete a Voluntary Caregiver Form

Note:  If applicable, please provide any documentation for additional services such as Special Education, 504, Gifted and Talented, or Dyslexia.
